Perk Test Request

Use the following form to request a perk test be performed by our company.

Perk test cost $350.00 (prices vary for commercial and vacant property and are controlled by the county) and are normally included in the price of your septic installation and or repair. However you may order a Perk Test without requesting any further services from us.

If you have contacted Miss Dig enter the 'Dig Ticket Number' in the area provided, otherwise enter none.

A Sidwell Number, Property Number and Tax Identification Number are all universally the same. To locate this number the best place to look is on your tax bill, where it will be listed by one of the three numbers shown above. Other sources to obtain this number include: Property Listings, Township/City Offices, Property Deed.

Please enter your response as numbers only. Example: 1275 should be entered for a home that is 1275 sq. ft. etc.

If this is a Residential perk you should enter the total number of bedrooms in the dwelling. If this is a Commercial Perk enter the total number of facilities (restrooms) in the building. If this is for a vacant parcel enter N/A or the total number of projected rooms (for new construction).

The county establishes the final date for the perk but we do try to accommodate your needs whenever possible. Please give the date which you expect to have the work done, note we usually perform perks on Thursday, but this is not etched in stone.
